Start preparing for your trip four to six weeks before you go.

As a surgery we provide a private clinic for travel vaccinations. Travel Clinic appointments are a private service and not part of the NHS, though appointments with our Nursing team are free of charge.

Haresfield Surgery do however charge an administrative fee of £20.00 for processing travel forms and to cover the cost of the extra administration time and nurse’s time required to run the clinic.

Please print and complete both of the below forms before your appointment. Form 1 is to be completed once for the trip, and form 2 needs to be completed by every patient travelling. This will be used during your travel clinic appointment.

Forms will not be processed and appointments will not be scheduled unless we receive the administration fee.

All vaccinations necessary for travel to other countries are given by our nurses. The practice is also a recognised centre for administration of Yellow Fever Immunisation - so if you need a Yellow Fever vaccination, please complete the Yellow Fever form below, too.
